Hi Everybody!
Enrollments with insurances were nearly completed, then the California Avenue office flooded, and I’ve moved four blocks away with flood repairs & a quick make-over in progress at the new location while waiting on enrollment applications to be completed for the new address. I still have an application pending for Medicaid, hoping to welcome AHCCCS patients this summer. The Medicare enrollments take up to 65 days to process, so everybody please stay connected where your coverage is accepted until we’ve got confirmed enrollments.

If you want to stay with your new PCP but just want a consult for a second opinion about diet, medications, lab results, or your treatment plan, I do offer consultations without treatment for $75 pre-paid for one hour.
If you need self-pay primary care without insurance:
New patient visits must take place face-to-face, take a full hour, and are $100 pre-paid,
follow up visits: half-hour appointment is $75 pre-paid, a one-hour appointment is $120 pre=paid.
