07/2024 to 03/2025: CareFirst Wellness Associates:Full-time salary primary care in Lake Havasu City until CFWAAZ was sold to Optima clinics. I was notified on 3/14/2025 that my position was no longer available as it would be absorbed by Optima’s staff from Bullhead, AZ.
2/2024 to 3/2024: WARMC Urgent Care: Bullhead City, AZ: locums brief contract of eight 12-hour shifts, contract ending 3/15/2024 via BAS Healthcare Registry: 7am to 7pm Urgent Care FNP using AthenaOne EMR.
11/2023 to 01/2024: Acacia Family Practice Group: started Nov. 29, 2024 for full-time work offer, began part-time to give notice at last job—full-time hospice weekend on-call; However, Acacia announced closure and closed Lake Havasu City location on January 31, 2024
Tebra EMR used. Full-time hours were delayed reportedly because company lost Medicaid/AHCCC contracts due to billing errors prior to my arrival. Then clinic was closed due to company’s financial emergency on Jan. 31, 2023.
07/2023 to 12/2023: River View Home Health and Hospice, Parker, AZ / Big River, CA: RN full-time, salary weekend on-call for local home health, and hospice nursing agency, providing RN services and assessments, Friday evening to Tuesday morning weekly. Aug. 01, 2023 to Dec. 31, 2023 in anticipation of planned increase to full-time at Acacia Family Practice. Phone visits included for hospice and home health patients.
